Involvement in Accidents                                 

As of June 10, 1998, Senate Bill 191 eliminates the requirement that drivers involved in accidents submit the state form (green form) to the state.

This legislation does not change any other duty or responsibility of persons involved in an accident. Drivers will continue to notify the law enforcement agency of accidents.

Obtaining Copies of Accident Reports

To obtain a copy of your accident report, which the officer prepared, you may come to the traffic accident report office located in the Police Department on the 3rd floor of the Municipal Building, 3rd & Avery Streets, Downtown. Office hours are from 8:00am to 4:00pm Monday through Friday, with the exceptions of holidays.

There will be a $3.00 fee for a copy of the officer's report.

You may also obtain the officer's report by mail. Please send check or money order made payable to the "City of Parkersburg". The mailing address is:

With your request, please include the following:

  • Date of accident
  • Location of accident
  • Name of the driver

Be sure to include a business size (large) self-addressed, stamped envelope.
